Medical Case Management
At AIDS Care, every patient is assigned a case manager. Case managers can help coordinate medical services with on-site medical providers, improve medical compliance/adherence, and connect patients with community services like insurance, housing and food.
Case managers can:
- Help obtain and maintain medical insurance to limit the costs of care.
- Assist in obtaining appropriate Social Security or Department of Health and Human Services benefits.
- Provide support services, such as short-term crisis intervention and support groups.
- Design service plans to ensure that basic needs are being met.
- Connect patients with other service organizations and/or make referrals to an intensive community-based case manager, who can assist with:
- Beacon of Life Program (Latino Services)
- Securing safe and adequate housing, clothing and food
- Transportation to medical visits
- Advocacy in the community
- Massage and acupuncture services
- Legal services
- Domestic violence services
- Veteran's services
